Signs and Symptoms for identifying wound infection
Pus (always an indication of infection)
Heat
Erythema
Swelling
Induration
Purulent or increased exudates
Wound bed discoloration (yellow/ green)
Increased odor
Non-healing wound
Wound deterioration
Unexpected extension of wound
Increased tenderness
Tissue bleeds easily
Fever of 101° F or greater
Elevated white cell count
Cellulitis
Inflammation
Nodules or pustules often precede ulceration
Local or regional lymphadenopathy may be present
Sudden appearance of necrosis
Unhealthy coloration/dullness of previously healing tissues
